If you look beyond F-15s, we had convoys, tanks, A-10, F-15s, Tornado, etc shot down by friendly fire.

Not in the last 20 years. Total fixed wing aircraft losses from friendly fire during the entire Iraq war 2003-2009 was 2. And that war encompassed tens of thousands of missions, at minimum.

This is a colossal failure.

The less time you have to organize the more often is going to happen.

Agree with this, but I'd put it this way -- the more slipshod and incompetent the leadership, the more often things like this will happen. If they planned a mission over Kuwati airspace and didn't provide the IFF to the Kuwaitis, that is just massive stupidity and incompetence.

But there is no way a minimally competent operation loses 3 F-15s to friendly fire in a single day.
