Most accidents reported in the history of Tesla, 236 of them reported to the feds, 4 of them were fatalities. In all 4 fatalities FSD or Autopilot were engaged and in all 4 cases the public was not notified that FSD was involved Tesla only disclosed it in a federal filing.

Note not in this article but widely reported online the accident involving 49ers coach Kyle Shanahan was in FSD too, the media initially blamed the other driver then Shanahan came forward and said it was my fault I looked down to find my phone that had dropped and when i looked up I crashed. What he's NOT disclosing is the car was in FSD and apparently the cameras were blinded by the sun in Palo Alto.
