It wasn't just that KAL airline. It was also with the one that got shot down over Russia and others.
They found out that culturally the instinct to not correct their superior was more substantial than the instinct to survive so co-pilots did not correct the head pilot even if it meant their death.
They had to dramatically not only change the training but also the KAL culture.
