It has happened on one occasion, in a race for Washington State Treasurer, where two Repubs got nominated, in spite of more Democrats voting in the primary. This seems to be a rare outcome.
This system was supposed to yield less extreme candidates. It doesn't seem to have done that.
It may not be the best system, but the claims that it is a disaster seem a bit over-blown.
