To be a good/great FT shooter, and I'm not offering anything that you don't already know, that it's critical to the set up routine and execution process to be totally focused on the process 100% of the time and not deviate in any manner. Any breakdown of focus during that sequence (for whatever reason--distraction, pressure or just mental laziness) from set up routine through execution (from tip toes to holding follow through) will affect the accuracy of the shot. Shooting free throws highly accurately is more of a mental task than a physical one and it takes a lot of discipline and concentration/positive visualization to be a great free throw shooter.
Whether playing or coaching, I've always been a big proponent of positive visualization in all of the past sports activities I was involved in--baseball, basketball and golf. From reading your posts, I get the feeling that you approached things the same way.
